If you request company information through codebiotx.com we will collect Personal Information such as your name, address, phone number, e-mail address, survey information, website registrations or any other information that might reasonably be used to identify you individually.

 

We may collect Personal Information obtained from you on registration forms used to process your requests for services or information, such as contact information (name, address, e-mail address), password, username or code, age, date of birth, gender, ethnicity and medical condition.
